A. For new patients, please print and complete the new patient forms. Please bring your insurance card, any prescription medications, and your glucose meter if you are diabetic.
Q. How early should I arrive for my appointment?
A. Please arrive 10 minutes early for your appointment to verify and update any changes to your personal insurance information. You also need to allow time for parking due to construction at the hospital.

If you are a new patient please arrive 15 minutes early to your appointment to complete any necessary paperwork.
Q. Can I pay on my past due balance at my appointment?
A. Yes, we accept cash, personal checks, Visa, MasterCard, and Discover cards.
Q. What is your fax number?

Q. Do you send office notes to my primary physician?
A. Yes, we send the office notes for each visit to your primary care physician to allow them to be involved in your care. You do not have to sign any additional medical records releases for this to happen.
Q. If I am scheduled for lab work at the hospital can I go anytime that day?
A. Yes, you can go to the lab anytime the day you are scheduled from 7 a.m. - 6:30 p.m. You will get a reminder call that says that your labs are at 8 a.m., but you can go anytime the lab is open.
Q. If I have Medicare and I need to go to the hospital lab, do I need to come to the clinic first?

A. Yes, if you have Medicare you need to come to the office prior to getting your labs done to answer the Medicare questions. This only applies to Medicare patients who are getting their labs done at the hospital.
